IPPB Death Claim Settlement :
List of Documents :
A. Claim for Account WITH Nomination
1. Duly filled Death Claim form (Dont leave any field blank)
2. Death certificate of the deceased Account Holder
3. Photo & Address proof of the Nominee
4. Bank account details of the nominee
5. Stamped receipt for amount received signed by nominee
B. Claim for Account WITHOUT Nomination
1. Duly filled Death Claim form (Dont leave any field blank)
2. Death Certificate of the deceased Account Holder
3. Legal Heir Certificate / Succession certificate (In case of dispute/doubt)
4. Photo & Address proof of all claimants/legal heirs
5. Letter of Authoirty as per format – to be signed by all legal heirs & Attested by Magistrate/Notary public)
6. Notarized Affidavit as per bank’s format on Non-Judicial Stamp paper of Rs. 100/-
7. Letter of Indemnity as per Bank’s format on Non-Judicial Stamp paper of Rs. 500/- duly executed by all the claimants and sureties.
8. Stamped receipt for amount received signed by nominee Copy of all documents should be submitted with ORIGINAL for verification by branch official and all documents should be signed in presence of branch official. Important : Freeze the Account immediately on receiving intimation of the death of Account Holder Verify whether Nomination is registered or not.
Issue applicable Death claim form to the Nominee/Claimants. Inform & explain documents required as per checklist. Get all fields of the Claim form duly filled. No field should be left blank; Use ‘Not Applicable’ wherever required.
Get the claim form signed by all claimants & sureties in presence of BPM/SPM.
Verify copies of all documents submitted with original and attest as “Verified with Original & found Correct” with official Seal & Sign. Forward the Claim after verifying the correctness along with all required documents as per checklist to IPPB branch
